Three Key Things Practices Should Do to Prep for MIPS/MACRA

Providers have had two tracks of MACRA to choose from in 2017 – the merit-based incentive payment system (MIPS) and an advanced alternative payments model (APM). Most providers were, by default, placed in the MIPS program, and they put in a lot of hard work to avoid penalties during the uncertainty of MACRA’s first year. In 2018, there will be even more changes as we transition to full implementation by 2019 and beyond. Some of the new regulations for the second year include allowing providers multiple submission mechanisms within the ACI quality and improvement categories; an exemption allowance for practices with less than $90,000 in allowed charges for up to 200 patients in Medicare Part B; and the option of participating in MIPS as an individual or a group, including a new virtual group option.

As adjustments are being made to MIPS, action needs to be taken now, so providers can stay ahead of whatever curveballs are thrown. To some degree, physician practices will need to “build the plane while flying it.” The 2018 second-year rules were finalized on November 2, giving practices lead time to position themselves for success. After months of helping practices design and execute MIPS action plans, we’ve distilled the information gathered into the top three things medical practices can do to get ready for the 2018 MIPS/MACRA landscape.

1. Choose which measures to report on wisely.

Though there are over 270 MACRA measures available, many are specialty-specific, so not all will apply to your practice. Still, narrowing down which ones you will choose to report on can be complex. There are a few ways to approach this. First, it’s important to understand the definition of each measure. When choosing which measures to report on, your practice will be expected to report only on those that are applicable or relevant to the services you provide. Additionally, the Early Implementation Review: CMS’s Management of the Quality Payment Program offers valuable insights that can help your practice understand specific MACRA measures.

Once you’ve narrowed the measures down to only those that you could be eligible for, you can turn to your billing data to identify the most frequently reported procedures and diagnosis codes, and look for measures that include those. Another good way of choosing measures is through patient eligibility. Use the analytics tools in your EHR, PM, and billing systems to look at the data of your patient population and identify which measures would apply to them. It’s important to avoid those that have less than 20 eligible cases because they’ll receive an automatic minimum score, regardless of how good your performance is.

When choosing measures for your practice, it’s important to understand not just which ones are applicable to you, but also which ones you can do well with. Setting your practice up for success under the new MIPS/MACRA laws requires some upfront legwork that ultimately will help you mitigate the risk of a penalty and increase your chance of receiving an incentive.

2. Make sure your clinicians are on board.

The shift toward a value-based healthcare system addresses more than just patient satisfaction and practice revenue. There’s also a heavy, albeit indirect, focus on physician burnout. Put simply, when your physicians are overworked and spread too thin, they won’t be able to deliver the best care to their patients. This directly impacts your MIPS score. Before finalizing the measures you’ll report on, it’s important to get your clinicians on board. They will understand better than anyone else which measures are appropriate for them, and which will allow them to best demonstrate the quality of care they provide. Physicians who have gone through rounds of Meaningful Use policies may feel resentment toward this latest set of changes under MACRA.

The ability and willingness of your staff to adhere to value-based care and strive toward the final measures should be weighted heavily. To encourage engagement and ownership, consider having a brown bag lunch session and soliciting ideas for making the transition a success. Another way to engage your providers is to have them brainstorm ways to track and show progress on the measures they’ll be reporting on. For example, they may want to see weekly reports or monthly posters on the practice walls. The way in which your providers want to show progress should be amply supported as another way to encourage participation.

3. Ask your EHR or vendor for help.

Once your entire team is on board with the MIPS measures your practice will report on, you can work with your EHR vendor to determine what documentation is needed. Most will provide you with the support you need so that you can submit the minimal requirements to avoid a penalty, and prepare a plan for next year to achieve the incentive.

One of the most important things your EHR vendor can provide is a guarantee program of some kind. CareCloud offered a 2017 MIPS Guarantee, which ensured clients received full assurance of avoiding penalties during the uncertainty of MACRA’s first year. With the higher threshold of activity that will be required in 2018 and beyond, guarantees like this aren’t as easy to support. Still, you don’t want to carry a double risk by paying for a technology that can’t support your needs while you’re also trying to navigate a program that has a lot of potential for change. Though you may not be able to get a no-penalty guarantee, your EHR vendor should offer some kind of protection on the technology side.

We don’t know what’s ahead with MIPS/MACRA in the year building up to what should be full implementation. Though uncertainty exists, we can still be ready. Taking action now to prepare for pending changes can help your practice mitigate the risk of a penalty and, ideally, work toward earning an incentive.

HHS Secretary nominee Alex Azar appears before the Senate HELP Committee in the first of two confirmation hearings. Azar made it clear he would push to lower drug prices if confirmed, despite criticism he’d be disinclined to do so given price hikes for insulin and other diabetes drugs during his time as an executive at Eli Lilly. He evaded the question of whether or not pharma executives should be held personally accountable for matters like price fixing. Azar also stressed that, if confirmed, his top priorities will be addressing the opioid epidemic, Medicare innovation, and expanding access to affordable healthcare.

An AAFP follow-up study on physician acceptance of value-based payment models shows that acceptance and successful participation have increased since the study was first conducted in 2015. Fifty-four percent participate in some form of VPM, and 50 percent feel such models encourage greater care coordination between primary care physicians and specialists. A similar percentage are investing in health IT to better enable their value-based programs.

The US is in regrettably good company when it comes to extremely brief visits with the doctor. According to a new Reuters report, 18 countries offer average appointment times of less than five minutes (Bangladesh comes in at 48 seconds), while another 25 nations give patients just under 10 minutes. It’s not all doom and gloom, however. Appointment times on the US have increased 12 seconds each year, moving from 15 to 20 minutes in a comparatively short period of time. Researchers recommend looking into how that extra five minutes impacts patient health in terms of addressing preventative measures after dealing with acute concerns.

A JAMA Internal Medicine study finds that patient satisfaction plummets when physicians deny patient requests related to prescriptions and tests. The authors point out that physicians may have set themselves up for satisfaction failure by giving in to patient demands too many times prior, and should spend time thoroughly explaining their reason for denying the request.

A New Hampshire judge rules that 84-year-old Anna Konopka, MD cannot renew her medical license. She claims that state regulators forced her to give up her license over her unwillingness to use a computer and, as a result, her inability to comply with the state’s mandatory PDMP policies. The license hand-over also seems to be connected to a recent disciplinary hearing with the state licensing board brought on by several patient complaints of inappropriate treatment.

Google researchers develop software for the Google Pixel smartphone that can detect when a second set of eyes is looking at the phone’s screen. The researchers contend their gaze-detection software may be ideal for reading sensitive information or watching video in a public space.

Facebook will roll out its suicide-detection software to countries beyond the US, where it began piloting the technology in March. The AI-based software alerts specially trained Facebook staff in real time when a user posts suicidal messages, who then contact local authorities. First responders have checked in on people over 100 times in the last month based on Facebook software alerts.

#GivingTuesday: The AMA continues to accept donations for practices affected by this year’s spate of natural disasters. The AMA Foundation’s Physician Disaster Recovery Fund has already given out $75,000 to practices impacted by Hurricanes Harvey, Irma, and Maria; and $150,000 to general relief efforts in Florida and Louisiana.

The holiday shopping season is upon us once again, with Cyber Monday forecast to be the biggest online shopping day in US history. Adobe Analytics expects today’s Web-based shopping to total close to $6 billion, (versus Black Friday’s $5 billion). Unlike years past, I got out and about on Friday for several in-store purchases, but did the majority of my shopping online. My sole tech purchase (from Amazon) was an Ozobots coding starter pack, which I’m fairly certain family members of all ages will enjoy. I’ve seen them in action at several community STEM events and no one seemed able to resist taking a crack at their colorful coding. StickyJ Medical ID will provide TelaCare customers with medical ID jewelry that includes the telemedicine company’s ViewMyID website address and patient serial number for easy health-data lookup by first responders.

NPR looks at the role telemedicine plays in opioid addiction treatment, highlighting the challenges well-meaning physicians face when attempting to incorporate virtual visits into treatment plans that include many unbillable hours for counseling sessions and briefer check-ins. Jay Joshi, MD of Prestige Clinics (IN) occasionally uses telemedicine to prescribe addiction medication, but prioritizes face-to-face visits. “You’re not going to get a good system of healthcare for primary care in these high-risk areas unless you invest time and energy into these patients,” he explains.

This study reveals that consumers are increasingly turning to voice assistants like Alexa and Google Home for healthcare information and advice. Nearly 50 percent of the 1,000 respondents have used such technology to ask about symptoms (70 percent of users), wellness tips (59 percent), or to connect to a healthcare provider (48 percent).

University of South Carolina researchers find that a 2014 statewide mandate requiring all physicians to check the state’s PDMP before prescribing opioids to Medicaid patients has resulted in a 29-percent drop in such prescriptions. The mandate was expanded to include all patients earlier this year – a smart move given that CDC stats show that physicians in South Carolina write more opioid prescriptions annually than there are residents in the state.

The local paper looks at the unique things Midwest Regional Health Services (NE) is doing to retain its patient-centered medical home status and quality certifications. Efforts include posting physician quality measures on waiting room walls, organizing 12 week-long healthy living groups, and offering a ladies’ day that includes routine screenings, manicures, and make-and-go foot scrubs. The independent primary care practice has created consulting company Celtic Healthcare Solutions to help other practices transition to the PCMH model.

More Cyber Monday tidings: McAfee releases its list of most hackable gifts, with laptops, tablets, and smartphones taking top honors. Drones, digital assistants, and connected toys and appliances also make an appearance. The company has created a pretty cute campaign aimed at educating consumers about securing their digital gifts.

Folks have 18 days left to sign up for coverage at Healthcare.gov. Enrollment for this year is predicted (optimistically, some contend, due to the halved sign-up period) to top out at just under 10 million. If you really want to get into the weeds of ACA sign-up analysis, check out Chris Conover’s mind-bending arithmetic, which has lead him to conclude that, “Obamacare Exchange enrollments continue to perform well below expectations” (despite headlines that say otherwise).

Financial records released earlier this week show that HHS Secretary nominee and former pharma executive Alex Azar profited considerably during his time at Eli Lily. He was paid nearly $2 million during his final year at the company, and has amassed a financial portfolio valued at $20.6 million. After leaving Eli Lily at the end of last year, Azar went on to start healthcare consulting company Seraphim Strategies, valued at between $15,000 and $50,000. Azar is due to be confirmed November 29.

Blind triplet brothers Nick, Steven and Leo Cantos use smart glasses from Aira to “see” with the help of remote agents who narrate in real time what the camera-equipped glasses capture. The San Diego-based startup launched in April as a subscription-based service that offers 100 minutes of agent time with $89 a month.

An observational study involving 17 physicians and 42 patients suffering from overactive bladder finds that conversations about the condition were predominantly physician-centric, with physicians doing most of the talking and asking only yes-or-no questions that did little to advance patient understanding of the condition or its treatment.

University of Arizona Telemedicine Program staffers give thanks for increasing support from CMS for telemedicine, expansion of the Interstate Medical Licensure Compact, and FDA approval of telepathology technology developed by ATP founder Ronald Weinstein, MD in 1987. The university has been one of the first to incorporate telemedicine education into its medical school curriculum.

Family physicians share what they are thankful for ahead of Thanksgiving, with the majority pointing to the willingness of their colleagues to share their time and expertise.

The American College of Emergency Physicians offers tips on avoiding Thanksgiving-related trips to the ER. Last year, over 36,000 people went to the ER for knife injuries, food poisoning, burns, and heartburn and indigestion. I find it amusing, given the copious amounts of food and napping I plan on taking advantage of, that they include exercise safety tips for those that plan on participating in a “traditional Thanksgiving sporting event.”

This article reminds family members that talking about health histories over a hearty Thanksgiving meal is a great way to share concerns and proactively learn about issues that may crop up in the future. The strategy was first introduced by former US Surgeon General Richard Carmona, MD who dubbed Thanksgiving National Family History Day. HHS has created the My Family Health Portrait tool to assist family members with organizing and sharing their family’s health data.
