5 Questions With Pediatrician Ashley Moss, MD

July 9, 2017 5 Questions With Comments Off on 5 Questions With Pediatrician Ashley Moss, MD

Ashley Moss, MD is founder of Ashley Moss Pediatrics (MD), and vice president of the Montgomery Pediatric Medical Society. She opened her house call-based practice in January 2017, and now sees three to five patients a day.

image

What prompted you to launch a house call-focused practice?

I wanted to create a practice that was highly personalized and offered the best quality medical care for my patients. After working in a large traditional practice for nine years, I felt that the trend toward larger practices had hindered my ability to connect with my patients, and provide the continuity and individualized care that I felt was imperative to providing high-quality medical care. I had heard about similar house call-based practices in New York, California, and Texas and so I investigated and decided that not only was this type of practice feasible, it seemed like a really powerful model that could leverage new technology to improve healthcare quality, convenience, and outcomes.

What types of healthcare technology do you use on a daily basis?

I use an EHR that enables me to write notes while seeing patients in their home, email prescriptions to pharmacies, send lab orders and receive results.  I also use an electronic fax that interfaces with my Practice Fusion EHR.  I rely heavily on creative mobile devices that I can easily bring to the homes of patients. For well child check ups where vision and hearing screens are required, I use a handheld mobile device to perform visual acuity, photoscreening, and hearing screens.  The visual screening device then faxes the screening results to my EHR so that the vision screens are easily integrated into the chart. Finally, I use an online direct care practice administration platform from Hint Health that streamlines the signup process and billing for my patients.

What health IT is on your wish list?

I am considering purchasing a temperature monitor for my vaccine refrigerator that remotely monitors and records the temperatures inside my vaccine refrigerator and freezer. It also immediately alerts me by cell phone when either the refrigerator or freezer temperature is out of range. Currently, I keep handwritten temperature logs for the refrigerator and freezer.

I am also considering different telemedicine companies that would enable me to examine patients remotely via a virtual exam room. I’d like these telemedicine programs to interface more efficiently with my EHR; when I find one that does, I will probably decide to make that investment.

Are you participating in any type of value-based payment program?

I don’t participate in value-based payment programs as defined by insurance companies. I do provide my patients with high-quality medical care that is research- and experience-based, and delivered with genuine concern for each child’s wellbeing.

What are your thoughts on the role new business models like yours play in reducing physician burnout?

These models play a tremendous role in reducing physician burnout! Direct patient care practices enable physicians to do what they love and enjoy, which is establishing meaningful relationships with patients so that they can effectively care for them in a financially transparent manner. Physicians can keep patients healthier, provide better medical care, and avoid costly ER or hospital admissions when they know each patient individually. Both physicians and patients are happier with the delivery and quality of the medical care.

The latest investment figures from StartUp Health highlight the confidence investors continue to have in digital health. Nearly $4 billion has been invested in Q2 of this year (including 10 deals over $100 million each), putting 2017 on track to break several funding records. A total of $6 billion has been raised so far. Modernizinig Medicine’s $231 million funding round back in May is perhaps the most relevant to physician practices. While investments in EHR companies managed to stay in the top 10, the vertical saw the least market maturity in the first six months of this year compared to the flashier categories of patient/consumer experience, workflow, and analytics.

The results of a somewhat dated survey of 744 office- and hospital-based physicians in Rhode Island shows – surprise, surprise – no love for EHRs. Both types of providers had the same top issues with the technology – either they denigrated the quality of their interactions with patients, or negatively impacted the amount of time they were able to spend in the exam room. Though the survey was conducted in 2014, I can’t imagine the evolution of EHRs has lessened their concerns (technology-induced physician burnout seems to be on the rise, after all) – though some may have caved and hired scribes to aid with documentation workflow.

Remember when?: Pokemon Go celebrates its first anniversary. And while it hasn’t quite sustained the absurd amount of downloads it saw last summer, it’s still played on a monthly basis by 65 million people. For perspective, Uber has 40 million users a month, Pandora 77 million, and  Spotify 140 million. Though it didn’t have a huge impact in the world of healthcare, it’s less of a flash in the pan than I thought it would be.

A 20-month audit of Montana’s clinics for government employees finds that the facilities have failed to document improvements in outcomes, and have offered no evidence of ROI. The first of the state’s health centers opened in 2012, with five more opening and accruing costs in the $26 million-dollar range over the ensuing years. Auditors cited faulty record keeping and a “cumbersome” EHR system that did not allow for sharing of data between providers.

An Institute for Health Information Technology survey of providers in Georgia reveals top organizational challenges to be lack of interoperability, IT infrastructure costs, cybersecurity threats, lack of a trained workforce, and patient utilization of engagement technologies. (Surveyed vendors also cited connectivity and cybersecurity as top challenges.) Not to be deterred, respondents plan to adopt telemedicine, predictive analytics, and smart phone apps within the next two years. Georgia-based physicians are in a sweet spot when it comes to access to virtual consult expertise. The state has benefitted from telemedicine-friendly legislation over the last several years, and is home to the Georgia Partnership for Telehealth, which has helped to advanced telemedicine capabilities for providers in rural and underserved areas.

This article highlights the unethical lengths – including paying commissions to other physicians for referrals and redirecting patients to less-equipped facilities because of kickbacks -  many physicians in India go to in order to attract patients. The “cut practice” has become so widespread, especially among physicians who are just starting out, that it has spawned a public anti-corruption campaign and legislation.

President Trump nominates Indiana Health Commissioner Jerome Adams, MD as US Surgeon General. Vice President Pence appointed Adams to his post in 2014 during Pence’s time as governor. No word on whether Adams crossed paths with CMS Administrator Seema Verma during her time in Indiana’s Medicaid trenches. Adams, who according to his LinkedIn profile is a practicing anesthesiologist and associate professor of anesthesiology at Indiana University Health, will take over from Acting Surgeon General Sylvia Trent-Adams. She took over the position in April from Vivek Murthy, MD when Trump gave him the boot. Like Murthy, Adams – who came under fire for a county-wide HIV outbreak in Indiana attributed to dirty needles – will likely focus his public health campaign efforts on the opioid epidemic.

The US Patent and Trademark Office grants CSI a patent for technology related to the automated dispensing of prescription drugs as part of a telemedicine visit via CSI’s Managed Health Kiosk System. The company partnered with Doctor on Demand for telemedicine services in 2015, and marked 1 billion uses of its kiosks early last year.

Telemedicine

image

CareCloud adds telemedicine capabilities, including eligibility verification and scheduling tools, to its EHR and PM systems. It’s a smart move given that physicians seem well aware of the need to add virtual access to their practice, but have thus far been hesitant to spend money on additional technology with a yet-to-be determined ROI.

WellVia will integrate LowestMed’s prescription price transparency search engine with its telemedicine service, giving patients the ability to look up prescription drug prices during virtual consults.
